FENTANYL (FEN ta nil) treats severe pain. It may also be used to cause drowsiness before a procedure. It is often prescribed when other pain medications do not work well enough or cannot be tolerated. It works by blocking pain signals in the brain. It belongs to a group of medications called opioids.

Fentanyl Citrate 10mcg/1mL, Sodium Chloride 0.9%, Solution for injection

Cannabis

· Severity: Major

· Notes for Consumers: Discuss cannabis use with your care team. The effects of your medication may be altered if used with cannabis. Call emergency services right away if you experience slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, or trouble staying awake.

· Notes for Professionals: Advise patients to avoid cannabis use while taking CNS depressants due to the risk for additive CNS depression and potential for other cognitive adverse reactions.

Fentanyl Citrate 10mcg/1mL, Sodium Chloride 0.9%, Solution for injection

Grapefruit juice

· Severity: Moderate

· Notes for Consumers: Grapefruit juice and grapefruit-containing foods may increase the side effects of Fentanyl. Do not significantly increase grapefruit or grapefruit juice intake while taking Fentanyl. Avoid use if possible, as grapefruit and grapefruit juice can increase the levels of fentanyl in your blood, and increase the risk for serious side effects. If you notice you have difficulty staying awake, slurred speech, confusion, severe weakness, or difficulty breathing, immediately contact your health care provider.

· Notes for Professionals: Patients should avoid or limit their intake of grapefruit juice during fentanyl treatment. Fentanyl is metabolized by CYP3A4. Grapefruit juice may increase fentanyl concentrations and/or decrease systemic clearance by inhibiting CYP3A4, leading to increased or prolonged effects, including a risk for significant respiratory depression. If concomitant use is necessary, consider dosage reduction of fentanyl until stable drug effects are achieved. Monitor patients for respiratory depression and sedation at frequent intervals. Sudden discontinuation of a CYP3A4 inhibitor could decrease fentanyl plasma concentrations, decrease opioid efficacy or, possibly, lead to a withdrawal syndrome in a patient who had developed physical dependence to fentanyl

Alcohol

· Severity: Major

· Notes for Consumers: Do not drink alcohol while taking this medication. Drinking alcohol while taking this medication can cause serious side effects, such as central nervous system (CNS) depression, and can increase the risk of an opioid overdose. Call emergency services right away if you experience symptoms of CNS depression: slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, trouble staying awake. Talk to your care team about naloxone, a medication for the emergency treatment of opioid overdose.

· Notes for Professionals: Advise patients to avoid alcohol consumption while taking opioids. Alcohol consumption may result in additive CNS depression and may increase the risk for opioid overdose. Consider the patient's use of alcohol when prescribing opioid medications. If the patient is unlikely to be compliant with avoiding alcohol, consider prescribing naloxone especially if additional risk factors for opioid overdose are present.
